Wall of Honor Registration Instructions

Two methods are available for you to obtain a registration package and return the information to us. Please choose one of the options listed here.

Download Registration Package Online and Return by Mail:

Step 1 - Download the fillable PDF application; complete and print.

• Complete all applicable information, and be sure to sign under “Cardholder Signature” if contributing by credit card.
• Under 'Submitted by,' enter a number for 'Total number of pages...,' including profile and application forms and photographs.

Step 2 - Download the fillable PDF Honoree Profile; complete and print.

• Completing the honoree profile is optional, but encouraged. This is an opportunity for us to learn about the special qualities of your honoree.
• Prepare a profile form for each individual honoree.
• Indicate the order of preference (first, second, or third) for each honoree. This order should be the same as shown on the application. This is the order in which honoree names at the same giving levels will be engraved onto the Wall of Honor.
• When completing the profile form, please provide a brief description of the honoree’s achievement, involvement, or special interest in aviation or space exploration. Each honoree profile will be included in the National Air and Space Museum’s collection as a permanent tribute to those whose names are inscribed.
• Approximately 450 words can be typed in the Profile space provided on the fillable PDF honoree profile form.

Step 3 - Attach honoree photograph.

• For donations of $500 or more, you may include a color or black-and-white photograph (no larger than 5 x 7) of the honoree.
• This photo will be added to our collection along with the honoree profile and cannot be returned.
• We are not able to accept electronic formatted images of any kind. The photo must be a print copy.

Step 4 - Mail in completed application.

• Mail your completed application, honoree profile (optional), and photograph, if applicable, to the following address:

Receive and Return a Registration Package by Mail: If you experience difficulty downloading the registration package, or if you prefer to have a Wall of Honor application package mailed to you, please complete our online request form. Once you receive the package, fill it out and mail it or fax it to us using the instructions provided.

Can I mail the honoree profile and/or photograph separately from the Wall of Honor application? YES — Honoree profile(s) and photograph(s) can be mailed separately from the Wall of Honor application. All items should be sent to the mailing address in the Wall of Honor Contact Information page.

Please provide the donor name and address plus the honoree name on any additional correspondence sent after the original Wall of Honor application. This will assist us in matching up this new information with the original application.
