Wall of Honor Registration Instructions

The preferred method for registration is to print and mail the PDF registration form, then send your honoree profile and image (if appropriate) via e-mail to WallofHonor@si.edu. Step by step instructions and an e-mail checklist are provided here.

Step 1 - Download the PDF application, print and mail.

• Complete all applicable information, and be sure to sign under "Cardholder Signature" if contributing by credit card.
• If you wish to submit an honoree profile and/or photograph by mail, please see the second page of the application. However, the preferred method for honoree profile submission is via e-mail (please see step 2 below).
• Mail your completed application to the following address:

Step 2 - E-mail honoree profile and photograph (if applicable).

• Your name, address and phone number.
• Honoree name and Wall of Honor level.
• Honoree Profile
  Please include text within body of e-mail, maximum of 350 words.
• Photograph (for gifts at Sponsor level and higher)
  Please attach an image in JPEG format, no larger than 2000 pixels on a side, total file size no larger than 1MB.
• Photograph Caption/Details
  Please include date, name, location, credit and any other information about the photo, if known.
• Photograph Rights and Permission
  By sending a photograph, you represent that you have rights to the photograph submitted and that you give permission to the Smithsonian, National Air and Space Museum to use the photograph on the web or other materials relating to the Wall of Honor.
• Web Display Option
  If you do not want your honoree profile and photograph used on the National Air and Space Museum web site, please indicate this in your e-mail. Honoree profile materials are used in accordance with Smithsonian Privacy Policy.

Receive and Return a Registration Package by Mail: If you experience difficulty downloading the registration package, or if you prefer to have a Wall of Honor application package mailed to you, please complete our online request form. Once you receive the package, fill it out and mail it or fax it to us using the instructions provided.
